From: Danielle Bray, Graduate Student Benefits Advisor Subject: Graduate Student Employees – Fall 2007 Date: August 2007 This information is for Graduate Student Employees that will be reappointed, for the fall, and who elected to continue coverage over the summer, under the Graduate Student Employee Summer Continuation Health Insurance Plan. The student employee health insurance coverage will be automatically reactivated once the reappointment paperwork has been processed. Please have the student employee review their paycheck stub to confirm that health insurance premiums are being deducted by the following dates: -Research Assistants by November 9, 2007. -Teaching Assistants and Graduate Assistants by November 14, 2007. If the student employee does not see deductions by the dates indicated above please immediately contact Danielle Bray at 632-6144 to correct the situation. Please remember, if the situation is not rectified the student employee will experience problems with health insurance coverage. The student employee does not need to complete a new enrollment application unless he or she wants to make changes to their coverage (i.e. add or delete a dependent, change of address or cancellation of health insurance). These changes must be returned to the Student Health Insurance Office no later than September 25, 2007. A new health insurance enrollment form must be submitted if the student employee desires health insurance coverage, for the fall semester of 2007-2008, and he or she DID NOT enroll in the Summer Continuation Health Insurance Program or will change their title from TA/GA to RA or visa versa. The new health insurance enrollment form must be received, by the Student Health Insurance Office, within 30 days of their fall appointment date. The new biweekly rates for the health insurance coverage are as follows: Research Assistant: Individual coverage-$9.12 Individual + 1-$35.64 Family coverage-$51.59 Fellows: Individual coverage-$91.19 Individual + 1-$197.27 Family coverage-$261.07 TA or GA: Individual coverage-$4.79 Family coverage-$41.53 International Students that are not on the payroll system by September 28, 2007 will be charged the International Student Health Insurance for the fall semester. If the student employee has enrolled in one of the Graduate Student Employee Health Plans, adjustments to his or her SOLAR Account will be completed by October 31, 2007. Student Health Insurance: The Student Health Insurance Plan (SHIP) waiver deadline, for domestic students, is September 28, 2007. The instructions for Teaching, Graduate, Research Assistants and Fellows to complete the Student Health Insurance Plan (SHIP) waiver are as follows: 1. This must be done on-line through the students SOLAR Account. 2. After accessing the SOLAR Account home page, locate the link titled 'Student Financial Services' on the lower left of the screen. 3. Click on 'SHIP Request for Waiver.' 4. Complete the form by indicating your Grad Type as either a TA, GA or RA. Then go to DEPT ID and use the drop down box (in the shape of a small magnifying glass) to input your academic department. This will bring up the Description field. In this field begin with a percent symbol (%), then put the first three letters of your academic department, followed by another percent symbol (%). An example: for the Biology Department input %bio%. Next you will need to click on 'Look Up' to bring up a list of departments. Click on your department. Lastly, input your appointment start/end dates in the fields titled Assistantship Start Month and Assistantship End Month. Again use the drop down box and when the Month Display field opens do NOT put anything here, simply hit 'Look Up' and a list of months will appear. Click on the appropriate start and end months. 5. Submit the form (PLEASE WAIT! The system will provide an immediate acceptance message if the waiver was processed. If an edit message of any type is received, please be advised that the waiver was NOT processed and an additional attempt to complete the waiver should be done).
